Appointment Protocol

Screening protocol
  • Patients should contact the office they are seen in to schedule their appointment. The following screening protocol will be followed: 
  • Patients who are experiencing a fever, shortness of breath, cough, flu-like symptoms or loss of smell and taste or if a patient has no symptoms but has had close contact with someone suspected or known to have COVID-19 over the past 14 days they will need to speak with their MD prior to scheduling an appointment. These patients will not be seen in the office until they speak with an MD.
  • If you have a Chronic Illness PLEASE schedule an appointment!
Scheduling an Appointment
  • • When scheduling an MD appointment or an appointment for bloodwork, patients will be asked to provide detailed insurance information and given instructions pertaining to their appointment such as arriving on time and wearing a mask.

    • Outpatient Consent Form
    • HIPPA Notice
    • Acknowledgement of Financial Responsibility
    • New Jersey Immunization Form
Walk-in Appointments
  • Patients will need to schedule their appointments in advance. Walk-in appointments are not permitted.  
Visitor Policy
  • We ask all visitors accompanying patients to wait in their vehicle. 
Patient Arrival
  • Patients will call the office once they arrive in the parking lot. A Medical Assistant or Phlebotomist will greet them at the door where their temperature will be taken, the wearing of a mask is confirmed, and their hands will be sanitized. The patient will then be brought directly to an exam room or the lab. They will not stop at the front desk. Any paperwork that needs to be provided will be done so in the exam room or lab. All copays will be billed.
After the Visit
  • All instructions, follow-up information, and scripts will be either given directly to the patient in the exam room or left at the front desk by the physician which is where the patient can retrieve them and leave immediately.
Cleaning Process
  • All surfaces, equipment, light switches, and doorknobs will be disinfected on a regular basis. The Staff will be required to wear all necessary personal protective equipment.
