HOW TO ACCESS YOUR PATIENT PORTAL
The Patient Portal allows you, the patient, access to your health and medical information anytime and from anywhere provided you have an internet connection. You can view your appointment history and upcoming appointments. You can also review your health indicators, lab test results, immunizations, and a clinical summary.
The Patient Portal is secure and protects your personal information. It helps us involve you in your care and keeps you informed on your health.
To set up your Patient Portal:
- Provide us with your email address so we can set up your portal (call or visit your office)
- You will receive an email from "MDNet PHR" containing a registration link
- Use the link to create your password and access your Patient Portal, your username is your email address
- The link expires after 48 hours, so please create your password as soon as you can. Should the link expire, feel free to call us and we can send you a new one
- Access to your Patient Portal allows you to view past/future appointments, your health indicators, allergies, medications, documents and much more
